Dorothy's dress was made using the Simplicity pattern #4136. My only complaint with is it is that the most correct size of gingham wasn't available at the time, so I had to settle for making it with the "wrong" fabric.
The shoes on the other hand... I bought a lot of beads to do the shoes long before actually going back to the movie or realizing that the originals were done in sequins. Since I had no other use for 200g of red seed beads, I'm doing the positively crazy thing and hand-beading a pair of shoes anyways. As of 2009, one shoe is complete, and since it took me ~3 months to finish, the other one is temporarily on hold. I have made two pairs of more screen-accurate sequined shoes to wear in the mean-time, and I've also worn sparkly red sandals with the dress as a last resort (I seem to have major shoe-issues with this outfit!)
The wig for this one is styled on a base wig designed for pigtails - it has a skin part all the way down the centre of the back. I did the rolls and braids and am very happy with the results; I just need to curl the ends of those braids into ringlets for it to be perfect! (maybe once I get that second beaded shoe done)
